A randomized controlled trial assigns participants to comparison groups by chance to reduce systematic differences between groups. It can provide stronger causal evidence than uncontrolled observations, but its credibility still depends on concealment, masking, missing data, outcome selection, precision, and how directly the population and intervention match the claim.
What matters
- Randomization reduces confounding but does not repair every design flaw.
- A suitable comparator and concealed allocation protect the comparison.
- Prespecified outcomes reduce selective emphasis after results are known.
- External validity asks whether the result applies beyond the trial setting.
Why randomization matters
When assignment is truly random and concealed, known and unknown participant characteristics are less likely to be systematically different between groups at the start. That makes the group comparison more informative about the intervention.
Randomization is not the same as choosing participants randomly from the public. A trial can be internally careful yet enroll a narrow population that limits wider applicability.
The design details that protect the result
Allocation concealment prevents foreknowledge of the next assignment. Masking can reduce differences in behavior, co-interventions, measurement, and interpretation. A suitable control group helps separate the intervention from time, attention, expectations, and background care.
Attrition and missing outcomes can reintroduce imbalance. Readers should compare the number assigned with the number analyzed and inspect the reasons for loss to follow-up.
A randomized label is the beginning of appraisal
Check the registered primary outcome, analysis approach, confidence interval, adverse-event reporting, and whether the publication follows current CONSORT reporting guidance. Then ask whether the measured outcome is meaningful and whether the studied formulation matches the real claim.
One randomized study may still be imprecise, biased, or inconsistent with other trials. The wider evidence body determines confidence.
